The game uses side-scrolling city stages where the monsters climb buildings, punch through walls, and topple entire structures for points. Players can attack civilians and military units, but must also manage health while dodging tanks, helicopters, and soldiers. Each stage usually ends after enough damage is dealt to the city, then the action moves to a new location with increased pressure. On NES, two players can cooperate and split targets to clear stages faster.
